Alberta Health Services (AHS) wants to hear from families about routine childhood immunization. If you are the parent or guardian of a child or children between the ages of newborn and 15, please respond to the short survey before December 31, 2022 ahs.ca/childvaccine.
Results are strictly confidential and will be used by AHS to inform their childhood immunization process, as well as information Albertans need to support this important decision.
